Easy to show plan brochure with manual rates to provide employers and their employees. No quoting required. Enroll by traditional employee enrollment form or have the employer submit an electronic data form.
Plan features:
- For groups of 2 or more
- $10/$20 and $20/$20 co pays
- 12/12/24 service frequencies
- Husband & wife groups will qualify
- Voluntary rates and employer paid rates
- No DE9C submission
- No additional administration fees charged
